Program Funding Materials

This site contains all materials that you need to apply for consideraton of program funding. It will remain active year-round; however, we will indicate when we are accepting applications.

We are currently accepting applications for July 1, 2008 - June 30, 2009 contract cycle.

NOTE: To be eligible for consideration of funding, you must submit the following completed documents by December 31, 2007:

• Memorandum of Understanding
• Patriot Act Form

Applying for Consideration of Program Funding

Step 1

Download and submit the Intent to Apply document NO LATER THAN 11/15/07.

Step 2

Download the remaining documents to your computer, fill them out and send to Community Investment.

• Consolidated Budget
• Success Story ** (NOTE: This should NOT be attached to the application)

** When you submit the Success Story, you understand that:

1. You must have all signed releases on file.
2. These documents are used as public information/marketing materials by United Way of St. Joseph County

All items must be submitted to United Way of St. Joseph County by close of business January 25, 2008.
