This outcome measurement tool is designed to reflect the movement of agency clients on the path to self-sufficiency within each United Way funded program.

From the choices below, select the issue area under which your program is currently funded. This document will contain the life areas that are being evaluated in the programs that are funded in the respective areas.  If an area that you are measuring is not included on this spreadsheet, please notify Community Investment.

The completed report is due 30 days after the end of each quarter OR at the completion of the program run period (i.e. for 10 week programs.)
