Agency Manual

The new United Way of St. Joseph County Organization Affiliation Policy & Procedure Manual is now available on-line. This manual, approved by United Way of St.Joseph County's Board of Directors in April 2007, pulls together all necessary policies and procedures into one document. The content includes the following:

• Definition of the types of relationships between United Way and organizations
• Compliancy requirements and the process by which non-compliancy concerns will be addressed
• Eligibility requirements to be a partner with United Way of St. Joseph County

United Way Required Documents:

Each year, as part of your compliancy requirements you submit documents to United Way. They are as follows:

1. Memorandum of Understanding - this document must be signed by the Agency Executive Director and Board Chair and submitted to United Way by December 31 annually to be considered eligible for funding consideration. This document indicates that the Agency Executive Director and Board President understand all terms associated with being a United Way Partner and agree to comply with all terms.
2. Quarterly Financial Reports - directions, explanations and template are on this document. This is the ONLY document that United Way will accept for quartelry reports. If another non-United Way form is used, it will be returned for submittal of the correct document.
3. Quarterly Outcome Measurement Tool - the quarterly measurement tool is designed to communicate information regarding the movement of clients along the path to self-sufficiency.

To Access Any United Way Forms:

• Download the provided document and save it on your computer so that you may complete it at your convenience.
• Once you have completed the document, email it to United Way Community Investment.
